If you have not provided an NPC a single Nectar and the first heart under their name in the Codex has a purple bow, that means, after giving them Nectar, they will provide you with a Keepsake. Each heart represents one Nectar (or later on, Ambrosia) given to them by Zagreus. This can be determined by the number of red hearts below the character’s name. Early in the game, you can unlock the Codex of the Underworld, wherein you can check Zagreus’ relationship with other characters in the game.
